Sarah Reed Senior Living is Hiring a Registered Nurse - (3rd Shift) Near Erie, PA

RN Supervisor (3rd shift)
Position Summary
Directs nursing service activities in the building. Assigns duties to nursing personnel based on residents' needs, available staff and unit needs. Supervises and evaluates work performance in terms of resident care, staff relations and efficiency of service.
Qualifications
  • Gerontology experience preferred.
  • Graduated from an approved school of nursing and registered in the Commonwealth of Pennsylvania.
  • Continuing education as required to retain license and stay abreast of changes in the profession.
Essential Responsibilities
  • Monitors LPNs and all CNAs to ensure quality resident care. Maintains a positive attitude and sets a professional example.
  • Monitors that all available Nursing employees answer call bells and conducts call-bell audits.
  • Counsels and disciplines staff appropriately and re-trains and mentors to correct any inappropriate behaviors or procedures.
  • Promptly follows up with all new hires (and paperwork) and accurately assesses performance evaluations.
  • Works with the DON following up on problem areas.
  • Reviews resident’s condition changes and assists with Resident Assessments.
  • Ensures physicians, families and administration are notified whenever necessary.
  • Informs DON of all incidents requiring transfer to the hospital.
  • Makes rounds with physicians and ensures orders are obtained as needed and in conformance with established standards.
  • Performs venipuncture and IV therapy as ordered by physician.
  • Transcribes Physician orders when necessary and ensures that two staff members have noted them for accuracy.
  • Obtains report and gives verbal report to the oncoming RN Supervisor.
  • Passes on information needed for follow-up and signs the 24-hour report form as Supervisor.
  • Ensures the communication from the care plan meeting is updated on care plans.
  • Secures prescribed medications; performs direct services to residents such as administering medications and treatments, if deemed necessary. Disposes of medications per facility policy.
  • Review medication records for accuracy.
  • Recognizes and reports resident problems which will require referral to other community agencies.
  • Communicates effectively with Social Services department.
  • Follows up on recommendation by the Dietician and ensures weekly/monthly weights are done and accurate. If a 2 lb. weight loss or more resident must be re-weighed.
  • Attends inservices and any other related duties and responsibilities that may become necessary as directed by DON.
  • Monitors dining room and completes dining room QA at least monthly.
  • Ensures the staff is knowledgeable of diets and restorative feeding programs.
  • Obtains Physician orders for all new admissions/readmissions.
  • Coordinates a smooth admission and transfer process with staff and residents.
  • Upon discharge, plans and initiates resident and family health teaching and rehabilitation.
  • Ensures Incident Reports are documented in the computer and follow-up has been done; i.e., family notification, physician notification; and proper intervention is initiated.
  • Participates in QAPI meetings.
  • Provides tracking information for incident reports including interventions, psych medication changes, follow-up from previous meeting, fall/risk, care trac list and safety devices.
Non-Essential Responsibilities:
  • Direct care that can be done by other personnel when available such as bathing, feeding, ambulating, toileting.
  • Other related duties and responsibilities that may become necessary as directed by DON.
